Through our Care at Home portfolio, we are reshaping how acute care is delivered—extending hospital ‑ grade care into homes, communities and virtual settings, supported by digitally enabled, clinically safe and deeply human models of care. As part of the Bringing Care Home (BCH) strategy, we are scaling acute ‑ at ‑ home services nationally, including Hospital in the Home, virtual acute pathways and new models of acute substitution. This is a rare opportunity to help design and scale care models that reduce unnecessary hospitalisation while improving patient and clinician experience. We’re now looking for a Product Lead – Acute to own and grow this critical portfolio.
