As a BSO Team Lead your typical day will impact in the following ways: - Providing sound clinical knowledge and skills, acting as clinical expert and resource, and providing comprehensive, quality client/family-centered health care services. - Do site visits, as necessary, to assist in decision making for acceptance to the LTCH waitlist. - Aiding in the transition of long term care residents with responsive behaviours (new or existing) between community, hospital and neighboring long-term care communities (LTCHs). - Reviewing long-term care home applications as they are received by the community and making recommendations as to the communities’ ability to meet the client’s needs related to responsive/expressive behaviours. - Developing a comprehensive transi...
