Description This position is Hybrid role with work onsite and offsite and will require a flexible schedule which includes working evenings and/or weekends, if necessary. Using a team based approach, the care coordinator will act as a resource to patients and health care staff to ensure patients receive all the care and services they require. Care Coordinators must be proactive, well-organized, present a friendly demeanor and function well in a fast passed clinic environment while providing best patient care services.
