Position Overview: Helen Hunt Health Center is seeking a mission-driven Nurse Practitioner or Physician Associate. In this role, you will work with PCHC established patients and with new patients in a same-day acute care setting. Same day care is defined as care that is urgent and/or problem focused which cannot wait until the patient can see their primary care provider. Your team will include Helen Hunt Health Center’s outpatient Primary Care providers, other Care Team providers, Primary Care RNs, MAs, and other disciplines embedded in the practice but also include providers throughout the PCHC organization. The Care Plus provider will support increasing access to all patients by caring for new patients, seeking same-day care, and aid in seamless transitions of care during provider or service changes. · Providers will not have a panel of patients but will focus on acute outpatient primary care same-day visits. · Full -time 40 hours per week (4) 10 hr days. · Providers will rotate between Acute Care Setting (new and existing patient 2 days) and Care Team (established patients only) in the same-day service setting at Helen Hunt Health Center as needed.
