The primary role of the Housing Case Manager is the ongoing support of clients within a permanent supportive housing environment. The Housing Case Manager engages clients at the housing site and in the outside community to orient to the expectations of supportive housing, develop an ongoing Individual Support Plan, and to assist with linkage to needed community resources. The goal of the programs is to improve individual and community outcomes related to safety, holistic wellness, recovery, increase in skills and income, and housing stability. The Housing Case Manager plays an integral role in assisting clients in developing skills across a variety of life domains through teaching, linking, and coaching.
