Employment Type: Part time Shift: Day Shift Description: Position Purpose: Participate as a member of an inter-disciplinary team in the planning and implementing of home and community-based intervention with families and their children, from birth to age five that chose to participate in home visits voluntarily. Job Duties: Initiates, coordinates, and implements a referral and intake responsibility for Family Connections' new program participants. Assesses, plans, implements and evaluates the needs of families. Imitates, coordinates, and implements the development of the Individual Family Service Plan with program participants and those in attendance. Collaborates with community agencies, community programs, and families. Completes referrals for families as needed. Instructs, models, coaches, and provides ongoing encouragement and support to participating families in identified areas of family need. Hours: Part Time 30 hours/week Day shift: Minimum Qualifications: Require a Bachelor’s degree in Early Childhood, Nursing, Social Work, or related field.
