Hobart Medical Building 101 W 61st Ave Hobart, Indiana 46342 The Bereavement Coordinator implements planning, direction, and provision bereavement support services for all hospice patients and families to assist in meeting their emotional and spiritual needs during and after the patient's terminal illness.
