This dependable individual will prepare and attend IP meetings with DDS case managers. This individual will complete all necessary documentation and will maintain emergency fact sheets for individuals with developmental disabilities. The qualified candidate would possess strong interpersonal and communication skills, as well as good judgment. Attention to detail and strong organizational skills are a must. Full Time position available; Monday- Friday 37.5 hrs a week ESSENTIAL DUTIES & RESPONSIBILITIES: - Review and audits individual service plans (ISP) data, notes, health records and incident reports for accuracy and completeness. - Ensures IP packets are up to date and are received from outside agencies. Confirms timely receipt of staff training signature sheets. - Performs various tasks for IP meetings including meeting preparation, assists in collecting data and writing IP’s and follow up meeting minutes. - Conduct internal audits of programs and documentation to ensure adherence to applicable federal,...
