FTE: 0.80: Hours: 32: Shift: Days - 1st Shift: Position Summary: We are seeking a compassionate and highly organized Oncology Nurse Navigator to guide patients through the cancer care continuum. This role serves as a key point of contact for patients, families, and the multidisciplinary care team, ensuring timely access to care, coordination of services, and a seamless patient experience. The Oncology Nurse Navigator provides clinical expertise, education, and support from diagnosis through treatment and survivorship, while helping to remove barriers to care. Qualifications: - Coordinate and ensure seamless, evidence-based care across the oncology continuum - Conduct comprehensive assessments and collaborate with the multidisciplinary team to develop individualized care plans - Provide patient and family education, support, and advocacy, addressing clinical and psychosocial needs - Identify and address barriers to care to promote timely access and adherence to treatment - Develop and deliver education materials and outreach programs for patients, staff, and the community - Participate in quality impro...
