Franciscan Health Lafayette Central 1501 Hartford St Lafayette, Indiana 47904 *Full-time, Part-time and PRN positions available* The Home Health Hospice Aide, under the supervision of a licensed clinician, participates in the delivery of patient care in the home or inpatient setting. WHAT YOU CAN EXPECT: Assists patients with care and activities of daily living as directed in the established Plan of Care Communicates any change in condition or lack of patient progress. Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate. Provides assistance with personal hygiene and elimination needs according to the established plan of care. Schedules visits with the patient or caregiver.
