FlexStaff is seeking a Registered Nurse with CHHA or hospital experience for our client, a CHHA agency. This is a full-time, Monday-Friday, either 9a-5pm or 8a-4pm, depending on needs. Qualifications: You will be responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care. - License and current registration to practice as a Registered Professional Nurse in New York State. - Graduate of an accredited school of nursing, BSN preferred. - A minimum of one year experience in hospital and or minimum of one year experience in a CHHA preferred. Duties and Responsibilities: Coordinates cost-effective patient care that meets insurance and physician approval. Verifies insurance plan details and checks patient ID cards at home. Accepts case assignments based on client needs, regardless of location. Handles all CHHA requirements, including assessments and daily documentation. Travels to patients' homes/facilities to provide direct care. Admits patients promptly as needed. Completes Oasis in-person, accurately reflecting patients' clinical status, including diagnosis...
