Company: Endorsed Job Description: JOB SUMMARY The Intake Coordinator serves as a central point of contact for ECCMs palliative care patients and the care team, responsible for efficiently managing the patient appointment lifecycle from initial contact through post-visit follow-up. This role involves scheduling patient visits accurately and promptly within the Epic electronic health record system, handling inbound calls from patients regarding scheduling and general inquiries, and potentially assisting with a quick registration process. The incumbent will also be responsible for initiating appointment reminders, managing after-visit correspondence, and facilitating clear and timely communication with the supportive care team to ensure seamless patient coordination and high-quality service. ESSENTIAL RESPONSIBILITIES: - Obtain requests from provider or pharmacy via fax, provider portal or service form inquiry. May obtain requests from provider or pharmacy by phone in some areas of the organization. Use multiple software systems and various resource sites to determine member plans and requirements.
