The part time Cardiac Medicine Social Worker work with high acuity patients and their families on a cardiac medicine floor. Opportunity to utilize brief counseling skills to support adjustment to illness and patient engagement in their treatment. Work closely with physician, nursing, rehab and case management colleagues as well as community-based agencies to help facilitate transition to appropriate level of care and optimize treatment outcomes. Clinical supervision and professional development are provided and highly valued. Continuing education and Social Work Grand Rounds provided on site.
