As a Care Coordinator you will empower, uplift and help others heal from the many forms of mental illness, trauma or addiction. Those who have recently experienced crisis and/or hospitalization, may need additional guidance as they maneuver multiple systems, engagement with new treatment team members and adjust to changes outside of the clinical setting. To meet the increasing needs of our communities it is imperative that all team members understand and successfully reach and maintain the expected performance standard levels. In order to achieve total and sustainable success each person must to do their part.
