Oncology Social Services Care Coordinator

Job description

Job Summary: Provides supportive counseling/social work services to patients and families, acts as a liaison with other community resources. Assists staff and volunteers in understanding significant; social and emotional factors affecting a patient and family during advanced illness. Fulfills Cancer Committee requirements for Cancer Program accreditation. Essential Job Duties: Interprets program/services for patients and families, reviews referrals, and initiates psychosocial assessments to identify patient/family needs, resources, and acceptance. Participates in the Interdisciplinary Group (IDG) meetings assisting in the development of a Comprehensive plan of care; communicates and interprets patient/family needs and problems to the IDG. Maintains ongoing, supportive counseling with patients/families, assisting them with difficulties, and meeting social and emotional needs. Makes appropriate referrals to community agencies and/or community resources. Also advises staff/volunteers regarding services available to patients and families.
