Job Description: Home Health Clinical Success Mentor Supporting Des Plaines and Tinley Park Branches At Elara Caring, we’re looking for a highly experienced Skilled Home Health RN who is ready to step into a clinical leadership and mentorship role —guiding new clinicians, strengthening preceptors, and elevating care delivery across our branches. This is not a role for someone new to home health.
