Position Overview Provide support and collaboration with the special populations team by assisting with the coordination of program activities and day to day operations. Provide case management services to program participants. Primary Department, Division, or Unit: Primary Work Shift: Day Regular Scheduled Weekly Hours: 40 Compensation Range: $25.27 - $37.90 per hour, depending on experience Provide support and collaboration with the LGBTQI+ team by assisting with the coordination of program activities and day to day operations. Collaborate with Primary Care providers and the team in assessing, planning, coordinating, and managing the biopsychosocial needs of the individuals for care services and patients. Deliver support services to patients to improve patient care for the LGBTQI+ TGNCNB population. Provide case management services and utilize culturally appropriate material. Support the process of the multi-disciplinary collaborative patient-focused care; ensure the patient receives timely and unimpeded access to the appropriate, high-quality, cost-effective health care and social needs of the patients.
