Per Diem, Days Position Summary: The Physical Therapist provides physical therapy services to patients with various diagnoses as prescribed by a licensed physician. 1. Plan of Care / Delivery of Treatment a. Establishes a plan of care that corresponds to the proper diagnosis and functional limitations. b. Delivery of treatment is effective / appropriate and corresponds to plan of care. c. Identifies contraindications to the plan of care and informs the manager and physician as indicated. 2. Documentation a. Documentation is completed on time b. Documentation is legible. c. Documentation errors are very rare. 3. Productivity a. Units produced correspond to hours worked. b. Helps other therapist complete their case load prior to leaving work. c. Informs the manager when there are openings in the schedule. 4. Technical Knowledge / Skills a. Able to perform all job duties as described in the department specific competencies. b. Readily identifies contraindications to plan of care. c. Resourceful. Able to solve problems.
