DESCRIPTION OF POSITION: SUPERVISION: Reports directly to Director of Clinical Case Management - The Clinical Case Management RN Case Manager assesses the physical, functional, social, psychological, environment and financial needs of clients/patients; identify a cost -effective comprehensive plan to meet the families' service needs, and implement the plan.
