Part time (48 hours biweekly) Monday - Friday Varied Day/Evening Shifts Weekend and holiday rotation: 3, 8 hour shifts General Summary: Performs activities and duties related to the education of the patient and referral source regarding services along the rehabilitation continuum. Completes and documents thorough preadmission assessments of patients to determine and recommend the most appropriate post-acute care setting to meet the patient's needs for maximizing independence and health and to support the seamless of transition of patients along the continuum. Duties and Responsibilities: Essential Functions: - Collaborates with physicians, therapists, case managers and others in the early identification of potential candidates for inpatient rehabilitation. Establishes rapport and trust with all.
