WHAT YOU’LL DO: As a home care hospice p hysician, you will use your compassion , communication skills and deep expertise to build meaningful relationships with patients and families , providing the highest - quality care at the most challenging point in life. You will see the person behind each prognosis and create care plans centered around the patient’s w ishe s, needs, and beliefs , often working in concert with the patient’s primary care physician . Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach. Make visits in-person and via telehealth. Participate in Weekly team IDG meetings.
