Position Summary: This position is located at our Guthrie Home Care Office in Vestal, NY The Home Health Aide (HHA) will be responsible for providing personal care to patients, assisting with activities of daily living following a care plan developed by a registered nurse or a therapist. The HHA will have effective communication with patients and their families. The HHA will work closely with the patient's care team to provide seamless continuity of care. The HHA is responsible for following the aide care plan, reporting issues related to providing care and patient symptoms as directed in the care plan. Education, License & Certification requirements: PA/NY residents require High School Diploma or GED Required. Home Health Aide Certification Required. Or Has a Certification as a Nursing Assistant (CNA), then enrollment into Home Health Aide class will be arranged and covered by Guthrie Lourdes Hospice. Continued employment is dependent on successful completion and maintenance of the Home Health Aide certification.
