Reporting to the Clinical Care Manager, the Clinical Care Manager Assistant (CCMA) is responsible for assisting the CCM and the team nurses in ensuring the smooth functioning of the teams. The CCMA facilitates the timely completion of paper work and data entry relating to patient care documentation. The CCMA demonstrates knowledge of the EMR computer program and of proper documentation procedures. The CCMA demonstrates professionalism in speaking with patients and their families/caregivers, and with healthcare professionals and outside vendors. The CCMA takes responsibility for updating his/her knowledge of systems and procedures as these change. The CCMA supports the organizational vision and mission. EDUCATION AND QUALIFICATIONS: - Currently licensed in the State of Florida as a Licensed Practical Nurse (LPN), with a minimum of one (1) year of nursing experience .
