Position Summary: The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more. You’ll work as part of a multi-disciplinary team , using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.
