Job description

SUMMARY: Coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion. STATUS: Per Diem: LOCATION: ElderOne: DEPARTMENT: EO ADM CLINIC: SCHEDULE: M-F 4:30p-10:00p, Saturday and Sunday 8:00a-3:00p or 3:00p-10:00p EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
