Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team. Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes. Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.
