Summary of Performance Expectations: The Clinical Care Manager (CCM) coordinates patient care working synergistically with the inter-professional care team, the patient and family to achieve quality outcomes through efficient care. Provides evidence based culturally sensitive care, monitors patient and healthcare outcomes and applies best practice nursing strategies. Provides coaching to direct care staff related to clinical performance and measured outcomes. Participates in resolving unit issues or patient complaints. Leads and coordinates patient flow utilizing all resources both internal and external. Oversees patient experience using evidence-based practice. Provides input in the development and revision of policies appropriate to clinical practice and patient population. Designs and implements patient care strategies for disease specific populations as needed. Required: - Graduate of accredited BSN nursing program. - Current NC RN license or licensure from Nursing Compact State. - BCLS certification.
