Company: Providence at Home with Compassus Position Summary: The Chaplain provides professional and competent spiritual care ministry and is responsible for serving the spiritual, religious, and emotional needs of others in inpatient, outpatient, and community settings. As a member of the healthcare team, the chaplain is directly responsible for the implementation of the mission, values, policies, and procedures of the Department of Spiritual Health Services and the facility in which they serve. Position Specific Responsibilities: - Provide effective spiritual support and appropriate religious/spiritual resources that contribute to the well-being of care recipients, their families, and caregivers. - Provide spiritual care that respects diversity and differences, including but not limited to culture, gender, sexual orientation, and spiritual/religious practices. - Triage and manage crises in the practice of spiritual care. - Formulate and utilize individualized spiritual assessments, interventions, outcomes, and care plans; accurately and concisely document care in appropriate records. - Establish and maintain professional relationships, including collaborative relationships with co...
