Position Overview: We are seeking a compassionate and organized ACHD Registered Nurse Navigator to serve as a key point of contact for adults with congenital heart disease. The ideal candidate will coordinate care, support transition from pediatric to adult services, educate patients, and advocate for their needs across multiple healthcare settings.
