Job Summary: The MGH Department of Pharmacy is seeking a Clinical Pharmacist to join our Medication History/Transitions of Care team. This position will be focused on transitions of care, starting with admission medication histories, reconciliation, and supporting the continuum of care for patients from admission to discharge. The Clinical Pharmacist will supervise pharmacy technicians/students/residents participating in the daily conduct of this pharmacy program.
