Job Description Supports integrated operations and transitions of care activities. Coordinates patient placement with post-acute care facilities and/or services, transportation, and obtains insurance authorization to facilitate safe transitions of care. Communicates with all pertinent individuals or agencies to fulfil outstanding transition of care items identified in the patient's care plan. Performs assigned tasks to assist with quality care and patient throughput. Job Responsibility: 1.Follows standard policies, techniques, and practices to facilitate safe transitions of care, including but not limited to post-acute placement, transportation, and/or appointments, and obtain appropriate insurance authorization for the next level of care 2.Functions as liaison with all pertinent individuals or agencies that have an impact on patient transition of care 3.Uses team-based communication strategies to close the loop on referrals, hospital follow-ups, and any outstanding transition of care items identified in the patient's care plan 4.Performs responsibilities supporting the operations needs of Integrated Operations 5.Gathers and generates reports on various department metrics for revie...
