Job Summary The Social Worker LMSW (Licensed Master Social Worker) will provide generalist social work services across various care settings. This role involves assessing patient or outpatients’ needs, developing treatment plans, and providing therapeutic interventions. The LMSW may work in hospital, and outpatient settings, adapting their approach to meet the unique needs of each environment, collaborating with other healthcare professionals to deliver holistic care. Core Job Responsibilities: Patient Assessment & Evaluation: - Conduct thorough psychosocial assessments to understand patients' mental health, social, and environmental needs. - Identify and evaluate patients’ strengths, challenges, areas requiring intervention and barriers to effective care. - Makes recommendations to provider for treatment options and dispositioning.
