Our Company Rehab Without Walls Neuro Rehabilitation: Overview: Occupational Therapist (PRN) – 2 Days/Week Location: Everlasting Assisted Living Community | Phoenix, Arizona Setting: Neuro Rehabilitation | Rehab Without Walls Program Who we are looking for: Experienced therapist interested in working with a specialized neuro population - Provide 1:1 outpatient therapy in assisted living setting with access to on-site gym, outdoor therapeutic spaces, walking paths, gardens, and art rooms - You value autonomy, creativity in treatment planning, and patient-centered care What you'll receive: Flexible PRN schedule – approximately 2 days per week Opportunity to practice neuro-focused therapy with functional outcomes in your patients' lives - Paid per Hour (not just per visit) - Collaboration with a multidisciplinary clinical care team Responsibilities: What you will do: Responsibilities listed include but not limited to: - Perform evaluations and develop treatment plans - Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences - Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral...
