Description Salary Range: $49,419- $54,361 per year FLSA Status: Non-exempt: Schedule: Monday-Friday, 8:00 AM - 4:00 PM Job Summary: The Rapid Rehousing Stabilization Case Manager provides direct support to homeless individuals who are preparing for and transitioning to housing. This person empowers participants to take charge of their own life’s direction and gain sustainable independence, by operating from a “housing first” point of view. The Rapid Rehousing Stabilization Case Manager assists the participant directly or by referral to obtain and maintain stable income, apply necessary life skills, and assisting with access to services and connecting with outside community resources they need to live successfully in stable permanent housing. This person works as a team with integrated services at St. Francis House to stabilize the guest in housing, income and behavioral health.
