Job Summary: Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment. Essential Functions: - Communicates effectively with staff, residents and families. Assesses, develops, and maintains an individualized patient plan of care with the patient. - As needed, specified by SH entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS. - Communicates with other members of the health care team in order to ensure continuity of care and coordination of services.
