Overview NaphCare is hiring a Medical Discharge Planner to join our team of medical professionals in the Clackamas County Jail located in Oregon City, Oregon . We currently have a Full Time Days position available! Pay rates range from $27.00-$28.50 per hour. Pay rates range per hour based on employment status, site needs, licensure, and experience. As a Correctional Medical Discharge Planner, you will work alongside correctional officers and other medical professionals to assists incarcerated individuals with planning their transition back into the community. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs. Responsibilities: The Medical Discharge Planner is responsible for discharge planning for all inmates released from the facility.
