Great opportunity for New Grads!: This position is responsible for pharmaceutical direct patient care and medication management for ambulatory patients, development of interdisciplinary pharmaceutical education activities for health care professionals, participation in performance improvement activities, and development of sustained partnerships with providers across the Foundation Health Partners system.
