Job Summary: The licensed clinician provides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning. Focuses upon transitions in care for assigned patient populations. Provides screening, assessment, planning, problem resolution, resource management, social work services, therapy and crisis management as they relate to behavioral health care and discharge planning needs of the patient/family. Provides and acts as a liaison to facilitate a comprehensive and individualized plan of care among physician, patient and/or family support systems, community agencies and facilities, and other entities within patient system of care.
