Position is full-time and is in Bloomington, IL Health Center hours are Monday – Friday 8:00 a.m. to 5:00 p.m. and Thursday 8:00 am – 7:00 pm. FLEXIBLE SCHEDULING OPTIONS! Responsibilities: Provide high-quality, patient-centered, culturally sensitive mental and primary health care services. Assess patient needs and create care treatment plans. Manage patient care. Staff cases as appropriate. Provide direct medical prescriber services. Help develop programs. Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.
