The Role: Under the general direction of the Program Director, the Targeted Case Manager (TCM) is responsible for supporting the wellness and recovery goals by implementing targeted interventions designed to provide timely, high quality and efficiency care. The TCM is to provide access to services that would encourage clients to resolve problems that interfere with their independence or self-sufficiency as well as maintain themselves in the community. Responsibilities: What You Will Do: - Provides case management to Clients who are Health Home eligible, adults diagnosed with severe mental illness, adults with two services in an inpatient, outpatient, residential and detoxification substance use disorder (SUD) program within a year.
