As our Post Acute Care Liaison, you’ll be the bridge between our hospice team and the hospitals and post-acute settings that trust us with their patients. What You'll Do: You will be responsible for establishing and maintaining relationships with hospitals and post-acute care settings — including nursing homes, rehabilitation centers, and assisted living facilities — to promote our hospice services and drive patient referrals.
