Sun River Health is currently seeking a Part-time Patient Support Specialist to join our team in Brentwood, NY. Summary: Assists patients in navigating the health care system, links patients to medical care, provides education and support to promote engagement and retention in care. Utilizes motivational interviewing offering patient centered care to promote medical adherence. Assists patients in reducing barriers to care and improve health outcomes. Shares lived experience strategies with patients to address issues of chronic diseases. Participates in monthly PAWH workshops and activities.
