The WelbeHealth PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. It’s our mission to serve the most vulnerable seniors with better quality and compassion in a value-based model, and the Travel Registered Nurse (RN) Specialist directly guides this care as a primary voice on the interdisciplinary team (IDT). The Travel RN Specialist covers any nursing roles (e.g., RN Case Manager, RN Clinic Nurse) as needed in multiple markets. The Travel RN Specialist is accountable for providing nursing care to participants in the program as assigned. Essential Job Duties: - Take ownership of the quality of care and outcomes for assigned participant panel, working directly and in tandem with the PCP, utilizing clinic - Conduct PACE required nursing assessments (minimally every 6 months), educate participants and their support network on acute, chronic, and end-stage conditions, and perform urgent and routine virtual or in-person follow-up visits as determined by participants’ risk stratification and needs, and as a direct extension to the PCP - Develop and implement a discipline specific plan of care (POC) in collab...
