Responsibilities: Qualifications: - Main Office location: Kingston, PA - Service Area: Wilkes Barre area. - Job Type: Part-time - Schedule: Monday to Friday 5:00 pm – 8:30 am - Initiates appropriate preventive and rehabilitative nursing procedures. - Administers medications and treatments as prescribed by the physician. - Initiates the plan of care and makes necessary revisions as patient status and needs change. - Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM) - Assesses patient’s needs through triaging and providing in-home care when needed. - Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient. - Resp...
