Certified SUD Care Coordinator (RADT or CADC)

Job description

Location: Oakland,California,United States The Substance Use Disorder (SUD) Care Coordinator/Case Manager is responsible for developing and providing high quality therapeutic interventions and services in support of an individual’s wellness and recovery focused on people struggling with co-occuring disorders. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. Current RADT, CADC 1/SUDCC certification is required or must be obtained within 90 days. 4 years of experience in case management in field based settings, addiction treatment, addiction counseling, substance use treatment, or peer counseling required. This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Alameda County is required. Schedules include one weekend day. Either Sun -Thurs or Tues -Saturday.
