Performs biological and psycho-social assessments including psychiatric evaluation, annual psychiatric assessment, physical assessment, and referral. Makes diagnoses and develops recommendations regarding treatment. Provides for the review and monitoring of consumer medication plans. Ensures wellness measures are taken including blood pressure, height, weight, etc. is taken at each Medication Review. Provides crisis evaluation of children and/or adults who may be in need of hospitalization or who may need medication to avoid hospitalization.
