Licensed Social Worker (LSW) or Non- Licensed Social Worker We are currently seeking a licensed social worker plays a key role in enhancing the quality of life for residents by supporting their psychosocial well being, coordinating services, and ensuring smooth transitions throughout their stay. This position works closely with nursing, therapy, admissions, and families to advocate for resident needs and uphold regulatory standards. Key Responsibilities: - Complete psychosocial assessments, care plans, and documentation in accordance with state and federal regulations. - Provide emotional support, counseling, and crisis intervention to residents and families. - Coordinate discharge planning, including community resources, home care, and support services. - Participate in interdisciplinary team (IDT) meetings and contribute to individualized care planning. - Maintain accurate and compliant documentation within electronic medical records (EMR). - Assist residents with adjustment to facility life, including addressing concerns and advocating for resident rights. - Facilitate family meetings and communicate care updates professionally and compassionately. - Support admissions by welcom...
