What You Will Do: The successful candidate will be willing and able to: - Provide same-day warm handoffs from the medical and referral teams; deliver brief, evidence-based interventions, psychoeducation, assess risk, and provide problem-solving support within the PCBH model. - Collaborate with PCPs, medical assistants, and BH team members; participate in case consultations; communicate patient updates and treatment recommendations to the care team.
