SUMMARY: Under the direction of an NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician-ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values STATUS: Per Diem: LOCATION: Unity Hospital - 1555 Long Pond Rd, Rochester, NY 14626 DEPARTMENT: Physical Therapy: SCHEDULE: Day Shift/Weekends (at least two shifts per month), 8-hr shifts 8:00 a.m. - 4:30 p.m. RESPONSIBILITIES: Within standards of practice established for a PTA, completes and documents (within agency standards) related to patient care, including visit notes, renewal orders, and verbal orders. Includes progress or lack of goal achievement in all charting. Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician, and/or interdisciplinary team members regarding patient response to therapy and any concerns. Provides PT gait and ambulation training.
