Description Position Summary: The Clinical Nurse Coordinator guides patients and families through the hospice journey by evaluating eligibility, coordinating admissions, and collaborating with physicians and healthcare partners. This role also delivers education to patients, families, and community partners, supports staff training and orientation, and conducts clinical chart audits to maintain quality and compliance.
